Clinical research progress of CO2 laser assisted deep sclerectomy

摘要: 近年兴起的抗青光眼手术方式中,二氧化碳(CO2)激光辅助的深层巩膜切开术(CO2 laser assisted deep scleral incision,CLASS)以其微创、操作简便及并发症较少等优势逐渐被广泛应用于临床。CLASS手术为传统的非穿透性滤过手术(non penetrating filtering surgery,NPTS)的改良,通过CO2激光的辅助,大大降低了传统NPTS手术的风险及操作难度。CLASS手术的降眼压效果虽然不优于传统小梁切除术,但是其术后早期并发症的发生率、对于角膜内皮的保护及再次手术的可操作性都优于小梁切除术。同时,CLASS手术还可联合其他手术方式,为青光眼的防治提供新的思路和理论基础。(国际眼科纵览,2021, 45: 290-295)

Abstract: In recent years, CO2 laser assisted deep scleral incision (CLASS) has been widely used in treatment of glaucoma because of its advantages of minimally invasive, simple operation and less complications. CLASS surgery is an improvement of traditional non penetrating filtering surgery (NPTS). With the assistance of CO2 laser, the risks and difficulties of traditional NPTS operation has been greatly reduced. Although CLASS surgery is not superior to traditional trabeculectomy in reducing intraocular pressure, it is superior to trabeculectomy in the incidence of early postoperative complications, the protection of corneal endothelium, and the operability of reoperation. At the same time, CLASS surgery can be combined with other surgical methods, providing new ideas and theoretical basis for the prevention and treatment of glaucoma.
