眼科手术相关心血管事件并发症的研究进展

Research progress on cardiovascular event complications associated with ophthalmic surgery

摘要: 随着眼科技术的进步和麻醉方案的改进,眼科手术相关并发症的发生率已显著降低。尽管严重心血管并发症的发生概率极低但仍可能发生,眼科医生必须全面掌握相关知识,以便采取主动预防措施、及时识别症状并实施最佳治疗方案,从而最大限度改善患者预后。眼科手术引发的并发症中心血管事件最为常见,包括可能引发血流动力学不稳定和心脏骤停的眼心反射、可能导致肺循环障碍、呼吸窘迫及心源性休克的医源性血管空气栓塞(如眼静脉空气栓塞或全氟化碳综合征)和术后静脉血栓栓塞等。本综述结合循证医学证据对眼科手术相关的严重心血管并发症进行系统概述,探讨其发病机制、高危因素、典型症状和体征,并简要阐述临床管理策略,为眼科临床实践提供参考。

Abstract: With advancements in surgical techniques and improvements in anesthesia protocols, the incidence of ophthalmic surgery-related complications has significantly decreased. Although the probability of severe cardiovascular complications remains extremely low, they can still occur. Therefore, ophthalmologists must have a comprehensive understanding of these complications to implement proactive preventive measures, promptly recognize symptoms, and administer optimal treatment, thereby maximizing patient prognosis. Among the complications triggered by ophthalmic surgery, cardiovascular events are the most prevalent. These include the oculocardiac reflex, which can lead to hemodynamic instability and cardiac arrest; iatrogenic venous air embolism (such as ocular venous air embolism or perfluorocarbon syndrome), which may cause pulmonary circulation disturbances, respiratory distress, and cardiogenic shock; and postoperative venous thromboembolism. This review aims to provide a systematic overview of severe cardiovascular complications related to ophthalmic surgery by synthesizing the latest evidence-based medical findings. It will focus on their pathogenesis, high-risk factors, typical symptoms and signs, and briefly discuss clinical management strategies, offering a reference for ophthalmic clinical practice.
