眼部血管内皮瘤的诊断和治疗

摘要: 血管内皮瘤是一种主要发生于全身软组织、肝、肺、骨等部位罕见的交界性或低度恶性血管源性肿瘤,一般呈浸润性生长,偶尔也会出现转移。眼部血管内皮瘤为一种更为罕见的血管瘤类型,目前尚缺乏针对其良恶性的定位及统一的诊疗指南。眼部血管内皮瘤多原发于眼眶,也可原发于眼睑及泪器。临床表现为眼睑肿胀、眼球突出,影像学检查常显示肿瘤局部浸润生长伴眶骨壁破坏,全身转移较为罕见。病理改变多为血管源性内皮细胞增生,免疫组化显示内皮细胞因子CD31、CD34阳性。治疗方式以局部完整切除肿瘤为主,但术中应注意该肿瘤的出血倾向。手术切除辅以放化疗依然是原发于眼眶血管内皮瘤较为稳妥的治疗方案。(国际眼科纵览,2021, 45: 357-363)

Abstract: Hemangioendothelioma is a rare borderline or low-grade malignant angiogenic tumor, which mainly occurs in the soft tissue, liver, lung, bone and other parts of the body. It is generally infiltrative growth and occasionally metastasis. Ocular hemangioendothelioma is a more rare type of hemangioma, and there is no unified diagnosis for its characteristics and lack of treatment guidelines. Ocular hemangioendothelioma is mainly located in the orbit, but sometimes also in the eyelid and lacrimal organ. The clinical manifestations were eyelid swelling and exophthalmos. Imaging examination often showed local infiltration and growth of tumor with destruction of orbital bone, and systemic metastasis was rare. Most of the pathological changes were vascular endothelial cell proliferation. Immunohistochemical staining showed that CD31 and CD34 were positive. The main treatment is local complete resection of the tumor, but the bleeding tendency of the tumor should be paid attention to during the operation. Surgical combined with radiotherapy and chemotherapy is still a safe treatment for primary orbital hemangioendothelioma. (Int Rev Ophthalmol, 2021, 45: 357-363)
