角膜后表面屈光力对全角膜屈光力及屈光性白内障手术的影响

摘要:

测量角膜屈光力的仪器有手动角膜曲率计、IOL-Master、角膜地形图仪等,其原理包括模拟角膜屈光力、基于高斯厚透镜公式的角膜总屈光力、光路追击法获得的角膜总屈光力。角膜前后表面的相关性有限,若忽略角膜后表面屈光力,行白内障手术联合Toric人工晶状体植入,将造成顺规散光过矫,逆规散光欠矫。故个性化测量角膜后表面屈光力,得到精确的全角膜屈光力,对屈光性白内障手术的术后视力恢复至关重要。(国际眼科纵览, 2017,  41:   414-418)

Abstract:

The instruments for corneal power measurements include keratometer, IOL-Master,  corneal topography, and so on. The principles of the measurement of these instruments include  simulated keratometry(SimK), total corneal power based Gaussian optic formula(KGOF) and total corneal power obtained by ray tracing method(Kray). The correlation between the anterior and posterior corneal surface is limited. If we ignore the posterior corneal power, cataract surgery with Toric intraocular lens implantation will cause with-the-rule astigmatism overcorrection or against-the-rule astigmatism undercorrection. Therefore, it is essential to measure individual posterior corneal power and obtain accurate total corneal power for the recovery of visual acuity after  refractive cataract surgery.
