单中心神经眼科住院患者疾病构成比分析

Disease constituent ratio of hospitalized patients with neuro-ophthalmic diseases: A single center analysis

摘要:

研究神经眼科亚专业疾病分布规律及病种特征。设计 回顾性病例系列。研究对象 2166例神经眼科患者。方法 收集2015年1月至2017年12月解放军总医院神经眼科病区住院患者的临床资料,记录其疾病构成、年龄、地区分布情况并进行统计分析。主要指标 疾病构成比。结果 共收集2166例神经眼科患者,男女性别比为1:1.2,年龄(38.6±17.9)岁(1~87岁)。其中<18岁者348例(16.1%),≥18岁且≤60岁者1582例(73.0%),>60岁者236例(10.9%)。青少年儿童高发的疾病包括遗传性视神经病变,87例(4.0%),发病年龄(21.3±10.0)岁;外伤性视神经病变,85例(3.9%),发病年龄(24.0±14.5)岁。多见于老年人的疾病是缺血性视神经病变,293例(13.5%),发病年龄(53.3±10.0)岁。男性高发的疾病有缺血性视神经病变,293例(13.5%);外伤性视神经病变,85例(3.9%);遗传性视神经病变,87例(4.0%)。女性高发的疾病包括脱髓鞘性视神经炎,838例(38.7%);颅内静脉窦血栓形成,37例(1.7%);非器质性视力下降,21例(1.0%)等。病因排前三位的分别为脱髓鞘性视神经炎838例(38.7%),非动脉炎性前部缺血性视神经病变285例(13.2%),外伤性视神经病变 85例(3.9%)。结论 本研究中心神经眼科住院患者以中青年为主,病种复杂多样,视神经疾病尤其是脱髓鞘性视神经炎占比最大。(眼科,2018, 27: 381-385)

Abstract:

Objective To analyze the distribution and characteristics of different neuro-ophthalmic diseases. Design Retrospective case series. Participants 2166 patients with neuro-ophthalmic diseases. Methods Clinical data of hospitalized patients with neuro-ophthalmic diseases in department of neuro-ophthalmology in the Chinese PLA General Hospital from January 2015 to December 2017 were collected and the disease spectrum, age and living region distribution were analyze. Main Outcome Measures Disease constituent ratio. Results A total of 2166 patients with the male/female ratio of 1:1.2 were included in this study. The mean age at onset was 38.6±17.9 years (range 1~87 years). 348 patients (16.1%) aged below 18 years, 1582 patients  (73.0%) aged over 18 years and below 60 years, 236 patients (10.9%) aged over 60 years. The most common diseases in children included hereditary optic neuropathy (87 patients, 4.0%) with the mean onset age of 21.3±10.0 years and traumatic neuropathy (85 patients, 3.9%) with the mean onset age of 24.0±14.5 years. The disease that occurs mostly in old people was ischemic optic neuropathy (293 patients, 13.5%) with the mean onset age of 53.3±10.0 years. The most common diseases happened in men was ischemic optic neuropathy (293 patients, 13.5%), traumatic neuropathy (85 patients, 3.9%) and hereditary optic neuropathy (87 patients, 4.0%). The most common diseases in women included inflammatory demyelinate optic neuritis (838 patients, 38.7%), cerebral venous sinus thrombosis (37 patients, 1.7%) and nonorganic vision loss (21 patients, 1.0%). The three main etiology were inflammatory demyelinated optic neuritis in 838 patients  (38.7%) , nonarteritic anterior ischemic optic neuropathy in 285 patients (13.2%) and traumatic optic neuropathy in 85 patients (3.9%). Conclusion The patients with a great diversity of neuro-ophthalmic diseases are mainly young and middle-aged in this center. The optic nerve diseases, especially demyelinating optic neuritis, are particularly worth study in depth due to its high frequency. (Ophthalmol CHN, 2018, 27: 381-385)
