青光眼患者视觉参数与主观视觉损害及生活质量的关联性分析

Association of vision parameters with subjective visual impairment and quality of life in  patients with glaucoma

摘要: 目的 分析青光眼患者的临床视觉参数与其主观症状和生活质量评分的关联性。设计 横断面研究。研究对象 北京同仁医院眼科门诊就诊的101例原发性开角型青光眼患者的101眼(病情较轻侧眼纳入研究)。方法 对以上患者行最佳矫正视力(BCVA Log MAR)、视野(Humphery视野计,24-2标准模式)及对比敏感度(快速对比敏感度函数)检查。主观症状评价采用青光眼症状量表(GSS),生活质量评价采用青光眼患者生活质量-15项量表(GQL-15)。主观问卷与各视觉参数间关联分析采用Spearman相关分析。对于相关分析中具有统计学意义的参数进行进一步的多元线性回归分析。主要指标 GSS评分、GQL-15评分、视野平均缺损(MD)值、视野模式标准偏差(PSD)值、视野指数(VFI)、BCVA、对比敏感度。结果 本研究中101眼的视野MD值为(-5.59±5.30)dB ,平均BCVA(0.004±0.049)Log MAR,平均对比敏感度函数曲线下面积1.06±0.25。GSS评分与BCVA和12 cpd空间频率对比敏感度显著相关(P均<0.05),与其他视觉参数关联不显著。GQL-15评分与BCVA,视野MD、PSD值、VFI值,对比敏感度函数曲线下面积、对比敏感度敏度,1、1.5、3、6、12 及18 cpd空间频率对比敏感度均具有显著关联性(P均<0.05)。回归分析结果显示视野MD值(P=0.028),12 cpd空间频率对比敏感度(P=0.027)和BCVA(P=0.023)在矫正其他因素后均与GQL-15评分显著关联。结论 视野参数、最佳矫正视力、对比敏感度相关参数降低均与青光眼患者生活质量损害显著相关。(眼科,2025,34: 118-122)

Abstract:  Objective To analyze the association of various clinical visual parameters with the subjective symptoms and quality of life in glaucoma patients. Design Cross-sectional study. Participants A total of 101 eyes from 101 patients diagnosed with primary open-angle glaucoma at the Ophthalmology Outpatient Department of Beijing Tongren Hospital were included(the less affected fellow eye was included in the study). Method The patients underwent evaluation of best-corrected visual acuity (BCVA, LogMAR), visual field (Humphrey perimetry, 24-2 standard mode), and contrast sensitivity (quick contrast sensitivity function). Subjective symptoms were evaluated using the Glaucoma Symptom Scale (GSS), while quality of life was assessed with the Glaucoma Quality of Life-15 (GQL-15) Questionnaire. Spearman correlation analysis was conducted to examine the relationship between subjective questionnaire responses and visual parameters. For parameters that showed statistically significant correlations, further multiple linear regression analysis should be performed. Main Outcome Measures GSS score, GQL-15 score, visual field parameters, BCVA, contrast sensitivity parameters. Results In the 101 eyes, the mean visual field MD was (-5.59±5.30) dB, the mean BCVA was (0.004±0.049) LogMAR, and the mean area under log Contrast Sensitivity Function was 1.06±0.25. The correlation analysis showed that GSS scores were significantly correlated with the BCVA and the 12 cpd contrast sensitivity (all P<0.05), but were not significantly correlated with other visual parameters. The GQL-15 score were significantly correlated with the BCVA, the visual field MD value, the visual field PSD value, the VFI value, the area under log contrast sensitivity function, the contrast sensitivity function acuity, and the contrast sensitivity in 1, 1.5, 3, 6, 12, 18 cpd (all P<0.05). The regression analysis results showed that the MD value (P=0.028), the 12 cpd contrast sensitivity (P=0.027), and the BCVA (P=0.023) were significantly associated with the GQL-15 score after controlling for other factors. Conclusion In glaucoma patients, significant impairment in quality of life is associated with damage of visual field, best-corrected visual acuity, and contrast sensitivity.  (Ophthalmol CHN, 2025, 34: 118-122)
