Association between Chinese visceral adiposity index and retinal vein occlusion: the Kailuan Eye Study

摘要: 目的 探讨中国内脏脂肪指数(Chinese visceral adiposity index,CVAI)与视网膜静脉阻塞(retinal vein occlusion,RVO)的关系。设计 横断面研究。研究对象 开滦眼病研究中,RVO状态可判定且CVAI非缺失的受试者8499例(16998眼)。其中,RVO 111例(114眼),无RVO 8388例(16776眼),纳入右眼数据进行分析。方法 比较RVO组与无RVO组的基线特征。采用多因素Logistic回归分析CVAI与RVO的关系,进一步按年龄和性别进行分层分析,并采用限制立方样条Logistic回归评估CVAI与RVO风险之间的剂量-反应关系。主要指标  CVAI,RVO。结果 RVO组年龄更大、男性比例更高,糖尿病和高血压患病比例更高,收缩压、舒张压、体质量指数、腰围、空腹血糖、肌酐、尿素、腰臀比及CVAI水平均高于无RVO组(均P<0.05)。多因素Logistic回归分析显示,校正糖尿病、高血压病史和肌酐后,CVAI与RVO显著相关 [OR=1.371,95%CI(1.143,1.643),Wald χ2=11.619,P=0.001]。分层分析显示,CVAI与RVO的关联在男性和≥60岁人群中更为明显,但年龄和性别交互作用均无统计学意义(交互作用分别为Wald χ2=1.079,P=0.299和Wald χ2=1.196,P=0.658)。限制立方样条Logistic回归分析显示,CVAI与RVO风险之间的总体关联具有统计学意义(似然比χ2=12.099,P=0.007),而非线性检验无统计学意义(似然比χ2=1.447,P=0.485)。结论 较高CVAI与RVO存在独立统计学关联。CVAI与RVO风险之间未见显著非线性关系,其剂量-反应关系整体上更符合近似线性正相关。该结果提示,CVAI可能有助于识别RVO患者或高风险人群中的内脏脂肪异常及代谢-血管风险负担,并为系统性危险因素筛查和综合管理提供补充参考。

Abstract: Objective To investigate the association between the Chinese Visceral Adiposity Index (CVAI) and retinal vein occlusion (RVO). Design Cross-sectional study. Participants A total of 8,499 participants (16,998 eyes) from the Kailuan Eye Study with determinable RVO status and non-missing CVAI data were included. Among them, 111 participants (114 eyes) had RVO, and 8,388 participants (16,776 eyes) had no RVO. Only data from the right eye were analyzed. Methods Baseline characteristics were compared between participants with and without RVO. Multivariable logistic regression was used to assess the association between CVAI and RVO. Additional subgroup analyses were performed by age and sex. Restricted cubic spline (RCS) Logistic regression was further applied to examine the dose-response relationship between CVAI and the risk of RVO. Main Outcome Measures CVAI and RVO. Results Compared with participants without RVO, those with RVO were older, more likely to be male, and had higher prevalences of diabetes and hypertension. They also had higher systolic blood pressure, diastolic blood pressure, body mass index, waist circumference, fasting blood glucose, creatinine, urea, waist-to-hip ratio, and CVAI levels (all P<0.05). In multivariable logistic regression analysis, CVAI remained significantly associated with RVO after adjustment for diabetes, history of hypertension, and creatinine level [odds ratio (OR)= 1.371, 95% CI, 1.143-1.643, Wald χ2=11.619, P=0.001]. Subgroup analyses showed that the association between CVAI and RVO appeared more pronounced in men and in participants aged ≥60 years; however, neither the age interaction nor the sex interaction was statistically significant (Wald χ2=1.079, P for interaction=0.299 and Wald χ2=1.196, P for interaction=0.658, respectively). RCS Logistic regression analysis demonstrated a significant overall association between CVAI and the risk of RVO (likelihood ratio χ2=12.099, P for overall association=0.007), whereas no significant nonlinearity was detected (likelihood ratio χ2=1.447, P for nonlinearity=0.485). Conclusion Higher CVAI was independently associated with RVO. No significant nonlinear relationship was observed between CVAI and RVO risk, and the dose-response pattern was more consistent with an approximately linear positive association. These findings suggest that CVAI may help identify visceral adiposity dysfunction and metabolic-vascular risk burden in patients with RVO or individuals at high risk, and may provide supplementary information for systemic risk factor screening and comprehensive management.
