局灶性巩膜结节的临床和多模影像学特征分析

Clinical and multimodal imaging characteristics of focal scleral nodules

摘要:  目的  探讨局灶性巩膜结节(focal scleral nodules, FSN)的临床表现和眼底多模影像学特点。设计  回顾性病例系列。研究对象  收集2023年1月至2025年7月在北京同仁医院诊断为FSN的患者6例(6眼)。方法  所有患者均接受视力、眼压、彩色眼底照相、OCT、OCTA、FFA、ICGA、眼B超检查等。主要指标  眼底改变、OCT、OCTA、FFA、ICGA、眼科超声检查特征。结果  6例FSN患者均为女性。患眼最佳矫正视力为0.8~1.0。所有病灶均表现为单发、边界清晰的黄白色隆起结节,4眼位于后极部。OCT显示所有病灶均局限于巩膜层内,呈边界清晰的圆顶状结构,未见明确脉络膜受累。病灶周围脉络膜受巩膜结节向上推挤而逐渐变薄,该特征在病灶边缘处最为明显。OCTA显示病灶区域脉络膜血流缺失。FFA早期表现为弱或稍强荧光,晚期呈荧光着染;ICGA全程弱荧光。5例患者获得长期随访,其中1例患者病灶在32个月内由3.29 mm×2.53 mm×0.88 mm增大到7.83 mm×6.34 mm×2.07 mm,接受光动力治疗后仍持续进展;其余患者病灶稳定。结论  FSN表现为位于眼球赤道后部的单发、边界清晰的视网膜下黄白色隆起结节,其特征性OCT表现为局限于巩膜内的类圆形病灶及表面脉络膜视网膜受压变薄。多数FSN病灶长期稳定。

Abstract: Objective To investigate the clinical features and multimodal fundus imaging characteristics of focal scleral nodules (FSN). Design Retrospective case series. Participants Six patients (6 eyes) diagnosed with FSN at Beijing Tongren Hospital between January 2023 and July 2025 were included. Methods All patients underwent comprehensive ophthalmic examinations, including best-corrected visual acuity, intraocular pressure, slit-lamp biomicroscopy, indirect ophthalmoscopy, color fundus photography, OCT, OCT angiography (OCTA), FFA, ICGA, and B-scan ultrasonography. Main Outcome Measures Fundus findings and imaging characteristics on OCT, OCTA, FFA, ICGA, and ultrasonography. Results Six patients (6 eyes) with FSN were included, all of whom were female. The best-corrected visual acuity in the affected eyes ranged from 0.8 to 1.0. All lesions presented as solitary, yellow-white, well-demarcated elevated nodules, 4-eye lesions located in the posterior pole. OCT demonstrated that all lesions were confined within the sclera, appearing as well-defined dome-shaped structures without definite choroidal involvement. The overlying choroid was progressively thinned due to upward compression by the scleral nodule, with the thinning most pronounced at the lesion margins. OCTA revealed choroidal flow voids within the lesion area. On FFA, the lesions showed hypo- or slight hyperfluorescence in the early phase with late staining, whereas ICGA demonstrated persistent hypofluorescence throughout all phases. Long-term follow-up was available in five cases. One lesion showed marked enlargement (from 3.29 mm×2.53 mm×0.88 mm to 7.83 mm×6.34 mm×2.07 mm) over 32 months and continued to progress despite photodynamic therapy, while the remaining lesions remained stable. Conclusions FSN is characterized by a solitary, well-defind yellow-white elevated subretinal nodule located posterior to the ocular equator. The characteristic OCT features of FSN consists of a roundish lesion localized within the sclera, accompanied by compressive thinning of the choroid and retina overlying the lesion. Most FSN lesions remain stable for a long time.
