    Online medical services: an unignored opportunity in the development of ophthalmology

    Online medical services based on the internet as the carrier and the technical means include health education, medical information query, electronic health records, disease risk assessment, online seeking advice from doctors, electronic prescriptions, telemedicine and remote treatment and rehabilitation, etc. Ophthalmology has unique advantages in the online medical services. Most diseases in ophthalmology are based on imaging examination. The most image detection methods of ophthalmology are noninvasive and fast, and the image is small and convenient for the transmission on internet even mobile terminal. In order to ensure the quality and safety of ophthalmic online medical services, the specific contents, application scope in ophthalmology, and the standardization of a variety of ophthalmic imaging methods and its transmission on Internet need to be studied.     A preliminary study of screening disease spectrum in image reading center of teleophthalmology

    Objective To study the constituent ratio of disease?in ocular fundus image reading of teleophthalmology. Design Retrospective case series. Participants The same ophthalmologist continuous in Beijing Tongren Hospital reading center of teleophthalmology during March to April in 2015, evaluated patient data 1192 cases, from 54 Chinese different grassroots hospitals. Methods The report data of evaluation were exported with Daheng Prust remote reading database software, and analyzed with Excel and SPSS software. If the diagnosis of the screening was not explicit, another ophthalmologist would check the diagnosis data in the database. The disease constituent ratios in different age groups (<50 and≥ 50) and in different geographic location( the northern and southern of the Yellow River ) were compared. Main Outcome Measures The disease constituent ratio. Result In the all 1192 cases, ametropia was in 408 cases (34.3%), cataract in 273 cases (22.9%), hypertensive retinopathy in 228 cases (19.1%), normal fundus in 160 cases (13.4%), retinal arteriosclerosis in 114 cases (9.6%), diabetic retinopathy in 85 cases (7.1%), macular and the posterior proretinal membrane in 57 cases (4.8%), pathological myopia in 47 cases (3.9%), glaucoma in 29 cases (2.4%), suspected glaucoma in 27 cases (2.3%), branch retinal vein occlusion in 25 cases (2.1%), diabetic macular edema in 21 cases (1.8%), age-related macular degeneration in 21 cases (1.8%). The disease diagnosis in less than 50 year group (366 cases) and in 50 years and over group (826 cases) respectively were ametropia in 168 cases (45.9%) and in 240 cases (29.1%); normal fundus in 106 cases (29.0%) and in 54 cases (6.5%); cataract in 19 cases (5.2%) and in 254 cases (30.8%); hypertensive retinopathy in 23 cases (6.3%) and in 205 cases (24.8%); retinal arteriosclerosis in 4 cases (1.1%) and in 109 cases (13.2); diabetic retinopathy in 10 cases (2.7%) and in 75 cases (9.1%); macular and the posterior retinal membrane in 0 cases (0%) and in 57 cases (6.9%); glaucoma in 4 cases (1.1%) and in 25 cases (3.0%); branch retinal vein occlusion in 2 cases (0.5%) and in 23 cases (2.8%); age-related macular degeneration in 0 cases (0%) and in 21 cases (2.5%). The disease constitution in the northern of the Yellow River group (796 cases) and in the southern of Yellow River (396 cases) group were hypertensive retinopathy in 172 cases (21.6%) and in 56 cases (14.1%); glaucoma in 12 cases (1.5%) and in 17 cases (4.3%); posterior vitreous detachment in 13 cases (1.6%) and in 14 cases (3.5%). Conclusion In present teleophthalmology screening based on fundus images reading, 1/3 of the patients belong to the ametropia. The ocular manifestations of the common systemic eye diseases are hypertensive retinopathy, retinal arteriosclerosis, and diabetic retinopathy, etc. The common eye diseases were mainly cataract, macular membrane, pathological myopia, glaucoma, etc. Cataract, hypertensive retinopathy, diabetic retinopathy, macular membrane, glaucoma are more common in aged 50 years or more. Hypertensive retinopathy is more common in patients of the northern of the Yellow River. (Ophthalmol CHN, 2015, 24: 220-225)

    The effect of femtosecond laser-assisted LASIK and transepithelial photorefractive keratectomy on corneal higher order aberrations in myopia

    Objective To evaluate the effects of corneal higher-order aberrations of FEMTO LASIK and TransPRK for myopia patients. Design Prospective case control study. Participants One hundred thirty-one eyes of 73 Chinese patients with myopia and astigmatism whose spherical equivalent refraction ranged from -1.00 D to -6.75 D. Methods There were 68 eyes of 37 patients undergoing FEMTO LASIK and 63 eyes of 36 patients undergoing TransPRK using aberration free ablation with SCHWIND AMARIS excimer laser. Visual acuity, residual spherical equivalent, wavefront aberration were examined preoperatively, 1, 3 and 6 months postoperatively. Main Outcome Measures Visual acuity, manifest refraction, corneal higher-order aberrations including coma, spherical aberration and higher order aberration. Results On 1 month after surgery, the percent of postoperative UCVA better than or equal to preoperative BSCVA in FEMTO LASIK group was 75%, which was higher than in TransPRK group (47.62%) (P=0.001). On postoperative 3 and 6 months, the percent of postoperative UCVA better than or equal to preoperative BSCVA in FEMTO LASIK group was 86.76% and 91.18%, which were similar to that of TransPRK group (79.37% and 90.48% ) (P=0.26, 0.89). The residual spherical equivalent refraction in TransPRK group (0.13±0.35 D) was a little bit higher than that of FEMTO LASIK group (-0.02±0.41D). The coma of corneal in TransPRK group (0.25±0.10?m) was lower than that of FEMTO LASIK (0.30±0.17 ?m) on 1 month postoperatively (P=0.04). The coma in TransPRK group (0.28±0.10 ?m, 0.30±0.12 ?m) were similar in FEMTO LASIK (0.31±0.16 ?m, 0.35±0.11 ?m) on postoperative 3 and 6 months respectively (P=0.12 , 0.13). There were no significant differences between spherical and total higher-order aberrations on postoperative 1, 3 and 6 months. Conclusions Both FEMTO LASIK and TransPRK increase corneal higher-order aberrations. Coma was higher in FEMTO LASIK than in TransPRK group on postoperative 1 month. Corneal higher-order aberrations are comparable between two different surgeries on 3 and 6 months postoperatively. (Ophthalmol CHN, 2015, 24: 225-229)

    The quality evaluation of ocular images from primary hospitals in teleophthalmology

    Objective To analyze the quality of the image data from the primary hospitals in teleophthalmology. Design Retrospective case series. Participants 158 cases aged 60.7±15.8 years old, from 15 provinces, 38 primary hospitals, evaluated by one ophthalmologist in remote eye reading center of Beijing Tongren Eye Center in June 2015. Methods Patient data were exported as an Excel file with the software of Daheng Pusin Cloud Medical System. Data integrity and image quality of each patient were carefully checked. The rate of various quality defects in patient data were compared between <50 years old and 50 years old or more, between the north of the Yellow River and the south of the Yellow River. Main Outcome Measures The rate of various quality defects in patient data. Result In all 158 patients, 74 patients (46.8%) had at least one type of quality defects. The quality defect rate of chief complaint with lack of time length, incorrect position on fundus image, absence of external eye image, shadow on macular area, blurry peripheral image caused by small pupil, inaccurate focus on fundus image, image without cutting, incorrect position on external eye image was 13.3%, 12.7%, 8.9%, 7.0%, 5.7%, 2.5%, 1.9%, 1.3%, 1.3%, respectively. The difference of quality defect rate of patients data between aged <50 years old (36.4%) and 50 years old or over (49.6%) had no significant (P=0.175). The difference of quality defect rate of patients data between the north of the Yellow River (46.4%) and the south of the Yellow River (47.5%) was not statistically significant (P=0.888). Conclusion The result showed that currently nearly half of the image transmission of patients in the reading service of teleophthalmology exists varying degrees of quality defects, in which the incomplete chief complaint, incorrect position on fundus image, absence of external eye image, shadow on macular area are more common.?(Ophthalmol CHN, 2015, 24: 230-233)

    The constitution of chronic PACG in PACG with acute attack and its treatment effects

    Objective To determine the constituent ratio of chronic PACG (CPACG) and acute PACG (APACG) in PACG with acute attack, and compare the difference of the results of treatment. Design Case-controlled study. Participarts Consecutive PACG patients (159 patients, 159 eyes) with acute attack. Methods Subjects were recruited and received one of two treatment options: immediate laser peripheral iridoplasty (LPIP) or intraocular pressure (IOP)-lowering medications. After the IOP was controlled, all the patients received examination: IOP, fundus examination, vertical cup-to-disc ratio (VCDR), visual field test, A-scan, UBM, gonioscopy, endocellium cell cunt (ECC), etc. All patients were divided into two groups CPACG (VCDR≥0.6) and APACG (VCDR<0.6), and received laser peripheral iridotomy (LPI). There was no further treatment if IOP ≤21 mmHg and IOP-lowering medicine would be added if IOP>21 mmHg. Filtering surgery would be performed when medicine did not decrease IOP effectively. Main Outcome Measures VCDR, IOP, acute attack duration (AD) , peripheral anterior synechia (PAS) extent, visual acuity, ECC, anterior chamber depth (ACD), lens thickness (LT), ocular axis length (AL), means defect (MD) of visual field and IOP-control-ratio. Results APACG (VCDR<0.6) occupied 85% in PACG with acute attack and CPACG (VCDR≥0.6) 15%, however APACG occupied 88.8% and CPACG 11.2% in PACG with AD≤168 h. PACG with AD≤168 h were further analyzed. There was no significant difference in AD between CPACG 23 h (12, 72) and APACG 15 h (10, 52) (P=0.508). PAS extents in CPCG 8.0 (3.0, 11.5) clocks were wider than APACG 2.0(0.0, 6.0) clocks (P=0.004). There was no significant difference in ACD and LT, but AL in CPACG (22.31±0.72 mm) were longer than in APACG (21.73±0.98 mm )(P=0.004). There was no significant difference in IOP between CPACG and APACG at the treatment of the acute stage. 73.6 % (92/125) patients accomplished 1 year’s follow-up after receiving LPI. The IOP control ratio of CPACG [55.6 % (5/9)] was lower than APACG [79.5 % (66/83)] (P=0.104). Conclusions CPACG occupies about a tenth in PACG with acute attack. CPACG had longer axial length and wider PAS extent than APACG. CPACG had a worse IOP control with LPI than APACG. ?(Ophthalmol CHN, 2015, 24: 234-239)

    Objective To research?the relationship between the existence of?tumor associated lymphangiogenesis in uveal melanoma (UM) tissues with the?histopathological risk factors of UM. Design Experimental?study.?Participants 55?paraffin embedded eyeball tissues of 55 UM patients?with perfect archived?clinical data in Beijing Tongren Hospital??from 2009 to?2014. Methods The?specific lymphatic endothelial?markers: LYVE-1?and D2-40, and Elivision immunohistochemical staining method,?were used to detect the?lymphatic?tissue and analyze its relationship with patients’ clinical?pathological?data in 55 UM cases.?The tumor’s lymphatic microvessel density (LMD) and microvessel density?(MVD) using CD-34 antibody were analyzed. Main Outcome Measures Lymphatic?positive rate of different UM?tissues,?LMD,?MVD. Results In?36 cases (65.5%)?of the 55?UM?tissues the?lymphatic vessels were observed. The lymphatic positive rate of tumor involving the?ciliary?body (95.0%) was significantly higher than that non-involving(48.6%) , and of tumor?spreading?outside the eyeball?(93.3%) higher than inside(55.0%) , and of tumor with bigger largest tumor?basal diameter (>16 mm)(82.4%) than the smaller (≤16 mm)(38.1%) , and of?with epithelioid?cell type(88.0%) higher?than non-epithelioid?cell type?(46.7%).?The difference all was statistically significant (P<0.05). And the difference of the lymphatic positive rate of UM?tissues was not statistically significant between different age (P=0.187), different gender (P=0.452), different eye (P=0.663), whether with iris neovascularization(P=0.935), with the secondary retinal detachment (P=0.641), with disc involvement (P=0.121), with invasion into the sclera(P=0.284), and whether receiving preoperative radiotherapy (P=0.483). The lymphatic microvessel density was positive correlation with microvessel density of UM (r=0.917, P<0.001).?Conclusion Tumor associated??lymphangiogenesis is in 2/3 cases of UM,??which may?play a role in?the?invasion and metastasis of?UM.     【Abstract】 Objective To investigate the causes of unsuccessful surgical approach in traumatic cyclodialysis. Design Retrospective cases series. Participants 55 cases (55 eyes) of cyclodialysis underwent at least one time unsuccessful cyclopexy from 2002 March to 2014 July. Methods The clefts were confirmed with ultrasonic biological microscopy (UBM). According to the previous operation records, preoperative examinations and the findings of the re-surgical procedures, causes of surgical failure were classified into 4 groups: suturing with wider apart, inadequate cover of the cleft, uncorrected suturing site and pseudo-suturing. Failure rates in different hospitals performed surgery were also compared. Main Outcome Measures The constituent ratio of unsuccessful causes of cyclopexy. Results There were 73 eyes suffered from deferent failure cases. In all failure 73 eyes, 23 eyes (31.5%) were suturing with wider apart, 22 eyes (30.1%) inadequate cover of the cleft, 15 eyes (20.6%) wrong suturing site, and 13 eyes (17.8%) pseudo-suturing. The number of eyes with wrong suturing site between Beijing Tongren Hospital group and other hospital group were 1 eye (4.1%), 14 eyes (28.6%) (P=0.01). The number of eyes with pseudo-suturing between Beijing Tongren Hospital group and other hospital group were 8 eye (33.3%), 5 eyes (10.2%) (P=0.02). In all 55 cases, 54 cases (98.2%) presented hypotension, and 32 cases (58.2%) presented shallow anterior chamber. Conclusions Major causes of unsuccessful cyclopexy in traumatic cyclodialysis include suturing with wider apart, inadequate cover of the cleft, unsuccessful suturing site and pseudo-suturing. Wrong suturing site is a predominant concern in cases from primary ophthalmic units. ?(Ophthalmol CHN, 2015, 24: 246-249)

    【Abstract】Objective To analyze the effect of combined surgical approaches including cataract extraction, intraocular lens (IOL) and capsular tension ring (CTR) implantation as the main method for secondary glaucoma due to lens subluxation. Design Retrospective case series. Participants 12 patients aged 62.3±6.9 years old of secondary glaucoma due to lens subluxation from July 2011 to September 2013 in Beijing Tongren Hospital were included. Methods The medical records before and after the surgery were retrospectively analyzed. All cases were implanted IOL in the bag, 11 cases were performed phacoemulsification cataract extraction, one case extracapsular cataract extraction, 6 cases CTR, 7 cases goniosynechialysis with viscoelastic, 5 cases trabeculectomy, one case transscleral cyclophotocoagulation, 4 cases pars plana vitreous water bag aspiration during the surgery. The mean follow-up was 13.3±10.0 months. Main Outcome Measures Visual acuity, intraocular pressure (IOP) and complications. Result The visual acuity in last follow up was improved in 8/12 patients, the improved lines of visual acuity was 4.9±2.8. Unchange of visual acuity was in 3/8 patients. All patients (100%) got control of IOP (< 21 mmHg) without drug at the final visit. No serious complications occurred during or after the operation. Conclusion The finding of small sample size and short-term follow-up of this article demonstrated that combined surgical approaches including cataract extraction combined with IOL and CTR implantation is safe and effective for secondary glaucoma of lens subluxation.     【Abstract】Objective To study the feasibility of porcine fibrin sealant kit as substitution of intraocular filling for closing the retinal hole. Design Experimental study. Participants 45 Qing-zilan rabbits (45 eyes). Methods Vitrectomy combined with puncture in retina and injecting balanced salt solution underneath the retina was performed for establishing hegmatogenous retinal detachment model in each eye of 45 Qing-zilan rabbits. Retinal reattachment surgery and laser photocoagulation line around the hole were performed in all groups. Then porcine fibrin sealant kit (Guangzhou Beixiu Corporation) was used to close retinal holes and perfusion fluid was filled in vitreous cavity in the fibrin glue group (14 eyes), 30% SF6 was filled in vitreous cavity in the gas group (14 eyes), and single perfusion fluid was filled in vitreous cavity in control group (14 eyes). Indirect ophthalmoscopy, fundus photography, B ultrasonography, OCT, and histopathologic examinations were preformed at 1week, and 4 weeks postoperatively. Main Outcome Measures Rate of retinal reattachment. Results On the 4th week after reattachment surgery, 12/14 eyes (85.7%) retinal reattached in fibrin glue group, while 13/14 eyes (92.9%) retinal reattached in gas group. There was no significant difference statistically between these two groups (P =1.00). However, only 5/14 eyes (35.7%) retinal reattached in the control group. Compared with the former two groups, there were significantly statistical differences (P=0.009, 0.036). Conclusions The result of experiment in rabbits shows that porcine fibrin sealant kit can replace intraocular fillings in closing retinal holes temporarily. (Ophthalmol CHN, 2015, 24: 254-257)

    【Abstract】 Objective To observe the short-term effect of injection of botulinum toxin type A (BTX-A) in neck muscles for treatment in residual torticollis after strabismus surgery. Design Retrospective case series. Participants The 21 children with residual torticollis more than 3 months after superior oblique incomplete paralysis surgery. Methods BTX-A injections in neck muscles were performed in 21cases with residual torticollis after strabismus surgery. Before the injection, target muscles were fixed by surface markers. Injection dose is with 4-8 U/kg and administrated at 2-8 points. Head position was measured with head position measuring instrument. And the parents’ feelings about these children’ head position were recorded too. Follow up time is 3-6 months. Main Outcome Measures Measured head position and parents’ assessment of these children’ head position. Results 3 month after BTX-A injection, mild amelioration of torticollis were presented in 38.1% of children, and significant amelioration was 9.5%. According to parents’ assessment, 57.1% children had mild amelioration and 9.5% had significant amelioration. Conclusions Short-term observation suggests injection of BTX-A in neck muscles has a limited effect in treatment of residual torticollis. (Ophthalmol CHN, 2015, 24: 258-262)

    Objective To observe drug sensitivity and resistance of ocular isolated bacteria to commonly-used antibiotics. Design Experimental study. Participants 2271 positive samples collected between January 2007 to December 2013 in Beijing Institute of Ophthalmology, Beijing Tongren Hospital. Methods Ocular isolated bacteria was identified using ATB semi-automatic identification instrument with bacterial suspension. Ophthalmic antimicrobial susceptibility of culture-positive microorganisms was tested using Kirby-Bauer disk diffusion method. The results are judged by 2012 edition of CLSI standard. Main Outcome Measures Drug sensitivity and resistance of ocular isolated bacteria. Results Among 2271 bacteria samples, 1514 cases (66.67%) were Gram-positive cocci, 261 cases(11.49%) were Gram-positive bacilli, 28 cases (1.23%) were Gram-negative cocci, 468 cases (20.61%) were Gram-negative baccilli. The most common Gram-positive bacteria was Staphylococcus epidermidis, followed by Staphylococcus aureus, Streptococcus mitis and Streptococcus pneumonia. The most common Gram-negative bacteria was Pseudomonas aeruginosa. Staphylococci are sensitive to vancomycin. The detection rate of Methicillin-resistant Staphylococcus is 59.4%. The resistance rates of Staphylococcus, Streptococcus, Enterococcus, Corynebacterium and non-fermentative bacteria to ceftazidime are 65.1%, 22.1%, 92.3%, 34.4% and 26.1% respectively. 0.3% of streptococcus strains are not sensitive to vancomycin. The resistance rates of Enterococcus to commonly-used ophthalmic antibiotics are significantly higher than other strains; few vancomycin-intermediate strains are seen. The resistance rates of non-fermentative bacteria and Enterobacteriaceae to moxifloxacin are 27.3% and 23.9%, slightly higher than other drugs. Conclusion Most ocular bacteria have higher sensitivity to quinolones. Vancomycin-resistant strains are not detected in Staphylococcus. Higher resistance rates to Cephalosporin antibiotics are seen in most bacteria; the drug resistance of Enterococcus to most commonly-used antibiotics is high especially.     Objective To study the relationship of children allergic conjunctivitis with symptoms of dry eye. Design Retrospective cases series. Participants 30 cases (60 eyes) of children seasonal allergic conjunctivitis, and 20 cases (40 eyes) of children ametropia as control group were analyzed. All of them had no symptoms of dry eye. Methods All subjects were inquired disease history and examined including external eyes, anterior segment, breakup time (BUT), corneal fluorescein staining (FL) and Schirmer I test (SIt). FL score was evaluated from 0 to 12, and 0 score indicated no staining of cornea. Main Outcome Measures BUT, SIt and FL score. Results BUT in allergic conjunctivitis group and in control group was (7.35±2.46)s, (11.95±2.183)s, respectively ( t =9.569,P=0.000).SIt in allergic conjunctivitis group and in control group was (18.28±5.123) mm, (16.50±5.149) mm, respectively( t =1.702,P=0.092). The patient number of non 0 FL score in allergic conjunctivitis group and in control group was 13 cases (20 eyes), 6 cases (6 eyes), respectively (χ2=4.193, P=0.041). There was no significant correlation between the time length of disease and FL score, BUT in allergic conjunctivitis group. Conclusion There was the correlation to dry eye in children seasonal allergic conjunctivitis even without symptom of dry eye. The decreased tear break-up time and positive fluorescent staining were the main representations. (Ophthalmol CHN, 2015, 24: 268-271)

    The preliminary study is about teaching mode on optical-related imaging examination for fundus disease. We guide the students to learn the operation of imaging apparatus and analyzing imaging at the same period, offering different levels of cases on the basis of different background in theory and clinical experience for students in ophthalmology.     Research team teaching in ophthalmology is a task-oriented teaching method of ophthalmology, aiming to improve the research capacity of an academic research team. Every team member is encouraged to participate, and his role is rotated between teaching and learning. The teaching content is settled around the research project of ophthalmology, including experimental method, relevant theoretical knowledge, and project progression and quality control. Several forms of teaching-learning methods are included, such as one to one or one to many training, group meeting, reading club, and salon. Through those learning process, team members can develop their abilities needed for specific project, including the innovation ability, analyzing and problem-solving ability, thereby enhance the research efficiency and core competitiveness of the ophthalmological team.
