    Research team teaching method in ophthalmological practice

    Research team teaching in ophthalmology is a task-oriented teaching method of ophthalmology, aiming to improve the research capacity of an academic research team. Every team member is encouraged to participate, and his role is rotated between teaching and learning. The teaching content is settled around the research project of ophthalmology, including experimental method, relevant theoretical knowledge, and project progression and quality control. Several forms of teaching-learning methods are included, such as one to one or one to many training, group meeting, reading club, and salon. Through those learning process, team members can develop their abilities needed for specific project, including the innovation ability, analyzing and problem-solving ability, thereby enhance the research efficiency and core competitiveness of the ophthalmological team.     Objective To observe the changes of posterior corneal surface after transepithelial photorefractive keratectomy (T-PRK) and epipolis laser in situ keratomileusis (Epi-LASIK) for myopia. Design Prospective comparetive case series. Participants 134 eyes of 68 miopia patients were divided into T-PRK group (81 eyes) and Epi-LASIK group (53 eyes). Methods Posterior corneal elevation and Q-value were analyzed using pentacam before and 1 week, 1, 3 and 6 months after operation,respectively. Main Outcome Measures Posterior coneal elevation and Q-value. Results In the T-PRK group, at 1st and 6th month after surgery and in the Epi-LASIK group at 6th month after surgery, the posterior corneal elevation have a statistically significant backward displacement(all P<0.05). No statistically significant difference was found in the postoperative corneal Q-value in both groups before and after surgery(all P>0.05). There was also no statistically significant difference of postoperative corneal surface elevation and Q-value between the two groups (all P>0.05). Conclusion The posterior corneal elevation showing a backward displacement after T-PRK for myopia,while posterior corneal asphericity has no changed.

    Objective To explore the effect of functional tear film on constitutional pterygium. Design Prospective comparative case series. Participants 53 cases of monocular patients with primary pterygium in Nanlang Hospital of Zhongshan City from August 2012 to February 2013. Method  14 pterygium patients with stationary phase and 39 patients with active phase were included in this study. All patients were examined  with Schirmer test, tear film break-up time (BUT) and conjunctival impression cytology for golblet cell count. Logistic regression was used to analyze associated factors and ptetygium activity.  Main Outcome Measures  Schirmer test, BUT and conjunctival golblet cell density. Results The result of Schirmer test in stationary phase and active phase was (9.43±2.28) mm and (8.67±2.14) mm, respectively (P=0.267). While the result of BUT in stationary phase and active phase was (8.79±1.97)s and (5.05±2.14)s, respectively (P=0.000). The mean globlet cell density (numbers/10 fields) in conjunctival impression specimens in stationary phase and active phase was (38.79±2.83) and (32.74±3.30), respectively (P=0.000). The logistic regression analysis showed that only BUT could affect the activity of ptetygium. Conclusions The active state of pterygium is associated with ocular surface status of patients. The patients of pterygiumms in active phase suffer poor tear film compared with patients in stationary phase. (Ophthalmol CHN, 2015, 24: 44-46)

    【Abstract】 Objective To study the effect of the application of minimally invasive phacotrabeculectomy in patients with unresponsive acute angle-closure glaucoma. Design Retrospective case series. Participants 15 cases(15 eyes)with acute angle-closure glaucoma and coexisting cataract in Xijing Hospital between March 2011 to March 2013. Methods Preoperative topical corticosteroids, mannitol, methazolamide and paracentesis were applied to reduce intraocular pressure (IOP). A standard phacoemulsification was performed, followed by minimally invasive trabeculectomy with a fornix-based conjunctival flap (5 mm) and small sclera flap (3 mm). Main Outcome Measures Visual acuity, IOP, anterior chamber depth, inflammatory reaction, and bleb-related complications. Results Pre-operative average IOP was (53.13±8.82) mmHg. One week after surgery the visual acuity of all patients was improved significantly, and the IOP was (13.41 ± 3.42) mmHg. 3 and 6 months after surgery IOP was (14.65±4.21) and (15.35±5.48) mmHg, respectively. Inflammation in the anterior chamber subsided postoperatively at one week, inflammatory exudate was almost absorbed, and anterior chamber depth was significantly deeper at two weeks, with an average of (2.72 ± 0.38) mm. All 15 eyes had diffuse blebs or elevated blebs with microcystic changes in the conjunctiva between 3 months to 6 months postoperatively. Postoperative vision acuity was (0.92±0.48) at 6 months. No serious adverse complications were encountered during follow-up. Conclusion Minimally invasive trabeculectomy could reduce tissue injury and improve the safety of surgery, which is an effective method when combined with phacoemulsification in the treatment of patients with unresponsive acute angle-closure glaucoma and coexisting cataract.     Objective To investigate clinical characteristics and etiology of infantile cataract combined with vitreous abnormalities. Design Retrospective and non-controlled case series. Participants From January 2013 to July 2014 at Beijing Tongren Eye Center eighty-four children (114 eyes) aged 3-year old or younger who were diagnosed as cataract and detected vitreous abnormality by ophthalmic color Doppler flow imaging (CDFI) ultrasound were enrolled. Methods Charts of these patients were reviewed. Age, chief complaints, signs, CDFI examination and suggested diagnosis, proven diagnosis, past history, family history, and pregnancy history of their mothers were recorded. Etiology and clinical characteristics were analyzed. Main Outcome Measures Age, symptoms, course, signs, CDFI manifestations and suggested diagnosis, proven diagnosis. Results Age at presentation varied from 0 to15 months (2.7±3.0 months). Age at diagnosis was 2~20 months (7.8±4.2 months). Abnormalities were noticed by parents in 81 cases (96.4%) and detected by eye examinations in 3 cases (3.6%). The first noticed symptoms were leukocoria in 39 cases (46.4%), unable to follow objects in 16 cases (19.0%), strabismus in 10 cases (11.9%), and binocular asymmetry in 7 cases (8.3%). The cataract was complete type in 75 eyes (65.8%), posterior polar type in 38 eyes (33.3%), and membranous type in 1 eye (0.9%). They could complicate with nystagmus, strabismus, microphthalmia, shallow anterior chamber, secondary glaucoma, persistent pupillary membrane, tractional retinal detachment and coloboma of choroid. CDFI suggested diagnosis included persistent hyperplastic primary vitreous (PHPV) in 111 eyes (97.4%), retinal detachment in 3 eyes (2.6%). Confirmed diagnosis after surgery included PHPV in 76 eyes (66.7%), family exudative vitreoretinopathy (FEVR) in 14 eyes (12.3%) and congenital cataract in 24 eyes (21.0%). CDFI suggested diagnosis and post-operative confirmed diagnosis were consistent in 69.3% cases. Conclusion Patients who were diagnosed as infantile cataract presumptively with abnormal vitreous were mostly at large month-old and mostly noticed as leukocoria by their parents, which could complicate with a variety of ocular abnormality. Pre-operative CDFI examination could detect vitreous and retinal disorders. (Ophthalmol CHN, 2015, 24: 103-107)

    Objective To evaluate the effects of corneal higher-order aberrations of FEMTO LASIK and TransPRK for myopia patients. Design Prospective case control study. Participants One hundred thirty-one eyes of 73 Chinese patients with myopia and astigmatism whose spherical equivalent refraction ranged from -1.00 D to -6.75 D. Methods There were 68 eyes of 37 patients undergoing FEMTO LASIK and 63 eyes of 36 patients undergoing TransPRK using aberration free ablation with SCHWIND AMARIS excimer laser. Visual acuity, residual spherical equivalent, wavefront aberration were examined preoperatively, 1, 3 and 6 months postoperatively. Main Outcome Measures Visual acuity, manifest refraction, corneal higher-order aberrations including coma, spherical aberration and higher order aberration. Results On 1 month after surgery, the percent of postoperative UCVA better than or equal to preoperative BSCVA in FEMTO LASIK group was 75%, which was higher than in TransPRK group (47.62%) (P=0.001). On postoperative 3 and 6 months, the percent of postoperative UCVA better than or equal to preoperative BSCVA in FEMTO LASIK group was 86.76% and 91.18%, which were similar to that of TransPRK group (79.37% and 90.48% ) (P=0.26, 0.89). The residual spherical equivalent refraction in TransPRK group (0.13±0.35 D) was a little bit higher than that of FEMTO LASIK group (-0.02±0.41D). The coma of corneal in TransPRK group (0.25±0.10?m) was lower than that of FEMTO LASIK (0.30±0.17 ?m) on 1 month postoperatively (P=0.04). The coma in TransPRK group (0.28±0.10 ?m, 0.30±0.12 ?m) were similar in FEMTO LASIK (0.31±0.16 ?m, 0.35±0.11 ?m) on postoperative 3 and 6 months respectively (P=0.12 , 0.13). There were no significant differences between spherical and total higher-order aberrations on postoperative 1, 3 and 6 months. Conclusions Both FEMTO LASIK and TransPRK increase corneal higher-order aberrations. Coma was higher in FEMTO LASIK than in TransPRK group on postoperative 1 month. Corneal higher-order aberrations are comparable between two different surgeries on 3 and 6 months postoperatively. (Ophthalmol CHN, 2015, 24: 225-229)
