    Clinical observation of changes in posterior corneal surface after transepithelial photorefractive keratectomy

    Objective To observe the changes of posterior corneal surface after transepithelial photorefractive keratectomy (T-PRK) and epipolis laser in situ keratomileusis (Epi-LASIK) for myopia. Design Prospective comparetive case series. Participants 134 eyes of 68 miopia patients were divided into T-PRK group (81 eyes) and Epi-LASIK group (53 eyes). Methods Posterior corneal elevation and Q-value were analyzed using pentacam before and 1 week, 1, 3 and 6 months after operation,respectively. Main Outcome Measures Posterior coneal elevation and Q-value. Results In the T-PRK group, at 1st and 6th month after surgery and in the Epi-LASIK group at 6th month after surgery, the posterior corneal elevation have a statistically significant backward displacement(all P<0.05). No statistically significant difference was found in the postoperative corneal Q-value in both groups before and after surgery(all P>0.05). There was also no statistically significant difference of postoperative corneal surface elevation and Q-value between the two groups (all P>0.05). Conclusion The posterior corneal elevation showing a backward displacement after T-PRK for myopia,while posterior corneal asphericity has no changed.

     Objective To evaluate 1-year postsurgical safety and efficacy of canaloplasty for open-angle glaucoma (OAG) in adult patients. Design Retrospective case series. Participants 14 cases (17 eyes) of adult patients with OAG and with preoperative intraocular pressure (IOP) higher than 21 mmHg from Beijing Tongren Hospital in 2011-2012. Methods Canaloplasty was performed in all patients and evaluation was performed at baseline and 1, 3, 6, and 12 months postoperatively. Main Outcome Measures IOP, numbers of antiglaucoma medications and postoperative complications. Results The mean baseline IOP was 21.95±6.99 mmHg on a mean of 2.06±0.97 medications per patient. The mean IOP was 15.06±2.73 mmHg at 1 month, 15.34±2.53 mmHg at 3 months, 15.30±2.89 mmHg at 6 months and 15.23±2.64mmHg at 12 months  postoperatively(all P<0.05). Medication decreased to a mean of 0.59±1.12 per patient at 12 months (P=0.001). Surgical and postsurgical adverse events were reported in 12 eyes (79.60%) including hyphema (10 eyes), elevated IOP greater than 30 mmHg (1 eye), corneal stroma hemorrhage (1 eye), shallow anterior chamber (1 eye). All the complications were relieved after treatment. Conclusion Canaloplsty is a safe and effective surgical procedure to reduce IOP in adult patients with OAG. (Ophthalmol CHN, 2014, 23: 22-25, 36)

    Objective To investigate clinical characteristics and etiology of infantile cataract combined with vitreous abnormalities. Design Retrospective and non-controlled case series. Participants From January 2013 to July 2014 at Beijing Tongren Eye Center eighty-four children (114 eyes) aged 3-year old or younger who were diagnosed as cataract and detected vitreous abnormality by ophthalmic color Doppler flow imaging (CDFI) ultrasound were enrolled. Methods Charts of these patients were reviewed. Age, chief complaints, signs, CDFI examination and suggested diagnosis, proven diagnosis, past history, family history, and pregnancy history of their mothers were recorded. Etiology and clinical characteristics were analyzed. Main Outcome Measures Age, symptoms, course, signs, CDFI manifestations and suggested diagnosis, proven diagnosis. Results Age at presentation varied from 0 to15 months (2.7±3.0 months). Age at diagnosis was 2~20 months (7.8±4.2 months). Abnormalities were noticed by parents in 81 cases (96.4%) and detected by eye examinations in 3 cases (3.6%). The first noticed symptoms were leukocoria in 39 cases (46.4%), unable to follow objects in 16 cases (19.0%), strabismus in 10 cases (11.9%), and binocular asymmetry in 7 cases (8.3%). The cataract was complete type in 75 eyes (65.8%), posterior polar type in 38 eyes (33.3%), and membranous type in 1 eye (0.9%). They could complicate with nystagmus, strabismus, microphthalmia, shallow anterior chamber, secondary glaucoma, persistent pupillary membrane, tractional retinal detachment and coloboma of choroid. CDFI suggested diagnosis included persistent hyperplastic primary vitreous (PHPV) in 111 eyes (97.4%), retinal detachment in 3 eyes (2.6%). Confirmed diagnosis after surgery included PHPV in 76 eyes (66.7%), family exudative vitreoretinopathy (FEVR) in 14 eyes (12.3%) and congenital cataract in 24 eyes (21.0%). CDFI suggested diagnosis and post-operative confirmed diagnosis were consistent in 69.3% cases. Conclusion Patients who were diagnosed as infantile cataract presumptively with abnormal vitreous were mostly at large month-old and mostly noticed as leukocoria by their parents, which could complicate with a variety of ocular abnormality.     Objective To study the relationship of children allergic conjunctivitis with symptoms of dry eye. Design Retrospective cases series. Participants 30 cases (60 eyes) of children seasonal allergic conjunctivitis, and 20 cases (40 eyes) of children ametropia as control group were analyzed. All of them had no symptoms of dry eye. Methods All subjects were inquired disease history and examined including external eyes, anterior segment, breakup time (BUT), corneal fluorescein staining (FL) and Schirmer I test (SIt). FL score was evaluated from 0 to 12, and 0 score indicated no staining of cornea. Main Outcome Measures BUT, SIt and FL score. Results BUT in allergic conjunctivitis group and in control group was (7.35±2.46)s, (11.95±2.183)s, respectively ( t =9.569,P=0.000).SIt in allergic conjunctivitis group and in control group was (18.28±5.123) mm, (16.50±5.149) mm, respectively( t =1.702,P=0.092). The patient number of non 0 FL score in allergic conjunctivitis group and in control group was 13 cases (20 eyes), 6 cases (6 eyes), respectively (χ2=4.193, P=0.041). There was no significant correlation between the time length of disease and FL score, BUT in allergic conjunctivitis group. Conclusion There was the correlation to dry eye in children seasonal allergic conjunctivitis even without symptom of dry eye. The decreased tear break-up time and positive fluorescent staining were the main representations. (Ophthalmol CHN, 2015, 24: 268-271)

    From 2012, the training programs of the post-graduate students with professional degree were reformed in focusing on the clinical fundamental training in the department of Ophthalmology in Beijing Tongren Hospital. In the past, the 2-month ophthalmic centralize course was the main pattern of training for the post-graduate students. And now, the students attended 1-hour morning training course twice every week during the 1st year’s rotation period. Thus the theories could be combined with practice closely, and the students could have deep impression and master the clinical skills rapidly.

    Objective  To study the characteristics of pediatric optic neuritis for the better treatment and nursing for them. Design Retrospective case series. Participants 75 cases (124 eyes) with pediatric optic neuritis (<16 years) from Jan. 2010 to Dec. 2012 in General Hospital of PLA. Methods Retrospective analysis of clinical data of 75 cases (124 eyes) with pediatric optic neuritis was conducted, collecting and analyzing their predisposing factors, clinical features. The follow-up was for 6~42 months to get further analysis for their prognosis and outcomes. Main Outcome Measures Predisposing factors, clinical features, and prognosis. Results The mean age of scheduled patients was 10.67±3.57 years, with 56 females (74.67%) . 29 patients (38.67%) had causes before onset, and all patients had different levels of vision loss. 49 patients (65.33%) exhibited bilateral diseases. In total, 32 patients (42.67%) had eye pain or eye pain with movement. All children followed up exhibited optic disc pallor. 51.52% of patients had recurrent optic neuritis, but visual acuity had recovered to 0.5 in 70% of patients. The final diagnosis of these patients were 4 multiple sclerosis (6.78%) and 9 neuromyelitis optica (15.25%). Conclusion The clinical characteristics of pediatric optic neuritis were severe bilateral vision loss and complete recovery. Compared with developing multiple sclerosis, female and older patients were in the risk of developing neuromyelitis optica.

    Objective To observe the anterior chamber compatibility of three different chitosan membranes and explore the possibility of these chitosan membranes to be the carrier of corneal endothelial cells transplantation. Design Experimental study. Participants Thirty-three New Zealand rabbits. Methods Thirty healthy rabbits were divided randomly into three groups, different degree of deacetylation of chitosan carrier (group A, B, C was 63%, 74%, 95%, respectively ) was implanted into the anterior chamber of the left eyes, and the right eyes were blank control group. Other three rabbits were only performed paracentesis of anterior chamber in the left eyes as controls. The reaction of ocular anterior segment was examined under the slit lamp microscope at different times in different groups. Corneal thickness was measured at postoperative 1st, 2nd, 4th and 8th week. Corneal endothelium cell density and morphology were examined under the corneal endothelia microscope at postoperative 2nd and 4th week. Corneal samples were collected for the regular histopathological examination and hematoxylin-eosin staining to observe the inflammatory reaction at postoperative 1st and 3th month. Main Outcome Measures Corneal thickness, cell density, hexagon cells rate, cell size.  Results Different degrees of corneal edema, anterior chamber exudation, iris hyperemia were observed within 1 week in all three groups. Hypopyon, iris neovascularization, decreasing of membrane transparency were observed at 2nd week and nearly disappeared at 2nd month in group A and B. Hypopyon and iris neovascularization were not observed at 2nd week in group C. The corneal thickness was statistically significant thinner in group C than in group A and B only at 2nd week after operation (F=13.425,P=0.000). No significant differences were found in cell density, hexagon cells rate, cell size between three groups and normal rabbit eyes (all P>0.05). The histopathological examination showed that the inflammatory cells gathered around the membrane in 1 month, ranuloma and wrapping of membrane were observed at the 3th month in group A and B. Wrapping of membrane by inflammatory cells was not found in group C in 1 month. Only few inflammatory cells were observed at the contracting area between cornea, iris and membrane. Inflammatory reaction disappeared at 3th month in group C. Conclusion Chitosan membrane with 95% degree of deacetylation has better anterior chamber compatibility than other two membranes (63%, 74% degree of deacetylation).

     Objective To explore the application value of trypan blue lens capsule staining in cataract surgery in Vision Recovery Project. Design Retrospective comparative case series. Participants 84 patients (84 eyes) with severe cataract in “Vision Recovery Project of Millions of Poor Cataract Patients” in 2012-2013 were recruited. Methods  Patients were randomly divided into observation group(42 patients, 42 eyes)  and control group(42 patients, 42 eyes). Observation group received 0.1 ml trypan blue capsule staining before continuous curvilinear capsulorhexis during phacoemulsification, but control group were not stained. Main Outcome Measures State of intraoperative lens capsule staining, success rate of continuous curvilinear capsulorhexis, visual acuity, intraocular pressure, degree of anterior chamber inflammation, corneal edema and posterior capsular opacification. Results The lens anterior capsule of all patients in observation group was stained as light blue after the trypan blue staining. The success rate of continuous curvilinear capsularhexis was 97.6% in observation group and 78.6% in control group (P=0.003). The difference of visual acuity, intraocular pressure, anterior chamber inflammation, corneal edema after surgery were no statistically significant (all P>0.05). 3 months after surgery, significant posterior capsular opacification appeared in one case in observation group and six cases in control group (P=0.027). Conclusion The continuous circular capsulorhexis and phacoemulsification in Vision Recovery Project could become easier to perform after anterior capsule staining with trypan blue. It is necessary for the students to adapt to the increasing requirements of ophthalmic medical service and to contribute to our society in China.

    Clinical efficacy of total vitrectomy for retinal detachment second to giant retinal tear

    Objective To determine the effectiveness of total vitrectomy in the management of giant retinal tears(GRTs). Design Retrospective case series. Participants Seventeen eyes of 17 consecutive patients with GRTs were enrolled at Zhongshan Ophthalmic Center from Jul. 2003 to Jan. 2008. Methods All patients were performed total vitrectomy, peeling off the adhesion cortical vitreous, perfluorocarbon liquid, partial retinectomy, endophotocoagulation, gas-liquid exchange and silicone oil tamponade. Nine eyes with ciliary epithelium detachment underwent resection of the detached ciliary epithelium. Scleral circling was placed in one eye with serious PVR. Pars plana lensectomy was performed in one eye with complicated cataract. The following-up times were 24.5±8.5 months. Main Outcome Measures The postoperative visual acuity, retinal attachment and complications. Results Retinal redetachment occurred in two patients. One with the occurrence of a new tear, the other one with serious postoperative PVR leading to tractional retinal detachment. The silicone oil was uneventfully removed in 15 patients between 2 and 18 monthes. One case was loss to follow-up. The final best-correcting visual acuity was light perception to counting fingers in 2 eyes, 0.02~0.3 in 5 eyes, 0.5~0.7 in 6 eyes, ≥1.0 in 3 eyes, The postoperative visual acuity was better than preoperation(χ2=17.01,P<0.05). Conclusion There is a favorable effect on the retinal detachment associated with GRTs of total vitrectomy, partial retinectomy, endophotocoagulation and tamponade of silicone oil during 2- year following-up.

    Continuing Medical Education(CME) goes along with the career of a doctor and is the key for a doctor to uprise his professional level. However, there are lots of difficulties in CME in ophthalmology, such as diversity of subjects, complexity of contents, formalism, rapid updating of knowledge, interdisciplinary and operation training difficultly in young eye doctors.     The scientific research competence of clinical postgraduates is obviously weak. The goal of the nation to train clinical doctor with high-quality requires to improve the doctor’s scientific research abilities. To meet this goal, we believe the short-term training and practice for clinical postgraduate candidates on basic scientific research is still necessary. The project-based basic research is an appropriate training mode which can significantly improve the clinical postgraduate candidates’ scientific thinking and capability.     Objective To investigate the protective effect and its mechanism of Qizhen capsule on the retinal ganglion cell (RGCs) of the rat optic nerve crush model. Design Experimental study. Participants 72 healthy adult male SD rats. Methods The 72 rats were divided randomly into two groups: treatment group (36 rats) and control group (36 rats). All the right eyes were selected as the experimental eyes, using a micro optic nerve forceps with the clipping pressure of 40 g to crush the exposed optic nerve for 60 s at 2 mm from the optic disc. The left eyes served as normal controls. The rats were given drug by gastric perfusion 2 h after the crushing, and then once daily. The rats of treatment group were given 20% Qizhen solution with a dose of 2.5 ml/kg; control group rats were given saline with same does for consecutive four weeks. At the 28 days after injury, the rats were sacrificed, HE staining, the expression of Caspase-3 protein and Tunel were detected with 24 rats of each group. The rest 12 rats each group were used to detect the expression of Bax and Bcl-2. Main Outcome Measures The thickness of retina, the expression of Bax and Bcl-2. Results The average thickness of retina in Qizhen treatment group was (109.0±4.4) μm, and (101.8±7.6) μm in the control group(P=0.028). The difference of the expression of Bax was significant (t=1.089,P=0.028), but there was no significant difference of Bcl-2 (t=0.553,P=0.692) between the two groups. Conclusion Qizhen plays a protective role for RGCs in rat optic nerve crush model, decreasing the apoptosis of RGCs may through reducing the expression of Bax and Caspase-3.

    Association of iVue OCT Signal Strength Index with blur degree of fundus photographs in cataracts

    Objective To analyze the association of the iVue OCT Signal Strength Index with the blur degree of fundus photographs in cataracts and to explore the feasibility to evaluate the severity of the cataract using the index. Design Evaluation of diagnostic methods. Participants 162 patients (324 eyes) with cataract  at Beijing Tongren Hospital in 2013. Methods All cases were confirmed by slit-lamp microscope examination. Fundus photographs and anterior segment photographs were taken with the non-mydriatic fundus digital camera. The grade of fundus photographs was based on the blur degree. The ratio of the diameter of the pupil and corneal diameter was obtained according to anterior segment photographs. The Signal Strength Index was obtained by the iVue OCT. Main Outcome Measures The grade of fundus photographs, the grade of the Signal Strength Index, sensitivity, and specificity. Results  With the increasing of the fundus photographs blur degree, the Signal Strength Index of iVue OCT decreased (r=0.891, P<0.001).  The sensitivity of high Signal Strength Index  evaluating the 0 degree of cataract fundus photographs was 89% and the specificity was 78%. The sensitivity of the moderate Signal Strength Index evaluating the 2 degree of cataract fundus photographs was 88% and the specificity was 67%. The sensitivity of the  low Signal Strength Index evaluating the 4 degree of cataract fundus photographs was 100% and the specificity was 94%. There is no significant correlation between pupil size and the Signal Strength Index (P=0.989). Conclusion The Signal Strength Index and the blur degree of fundus image in cataract have a good correlation. The Signal Strength Index of iVue OCT could accurately evaluate the blur degree of fundus photographs in the mild and severe cataracts.

    【Abstract】 Objective To study the effect of the application of minimally invasive phacotrabeculectomy in patients with unresponsive acute angle-closure glaucoma. Design Retrospective case series. Participants 15 cases(15 eyes)with acute angle-closure glaucoma and coexisting cataract in Xijing Hospital between March 2011 to March 2013. Methods Preoperative topical corticosteroids, mannitol, methazolamide and paracentesis were applied to reduce intraocular pressure (IOP). A standard phacoemulsification was performed, followed by minimally invasive trabeculectomy with a fornix-based conjunctival flap (5 mm) and small sclera flap (3 mm). Main Outcome Measures Visual acuity, IOP, anterior chamber depth, inflammatory reaction, and bleb-related complications. Results Pre-operative average IOP was (53.13±8.82) mmHg. One week after surgery the visual acuity of all patients was improved significantly, and the IOP was (13.41 ± 3.42) mmHg. 3 and 6 months after surgery IOP was (14.65±4.21) and (15.35±5.48) mmHg, respectively. Inflammation in the anterior chamber subsided postoperatively at one week, inflammatory exudate was almost absorbed, and anterior chamber depth was significantly deeper at two weeks, with an average of (2.72 ± 0.38) mm. All 15 eyes had diffuse blebs or elevated blebs with microcystic changes in the conjunctiva between 3 months to 6 months postoperatively. Postoperative vision acuity was (0.92±0.48) at 6 months. No serious adverse complications were encountered during follow-up. Conclusion Minimally invasive trabeculectomy could reduce tissue injury and improve the safety of surgery, which is an effective method when combined with phacoemulsification in the treatment of patients with unresponsive acute angle-closure glaucoma and coexisting cataract.
